Welcome to the Enrollment Process
We're excited to welcome your family to Humpty Dumpty Childcare! To enroll your child, please download the forms below, complete and sign them, and return them to us — either in person or electronically. Your Complete Enrollment Package
All 7 documents below are required for enrollment. Three of them are fillable PDFs — you can type directly into them on your computer before printing or emailing. The others should be printed and completed by hand.

Filling Out Your Forms
Read the Parent Handbook first
All other forms reference the handbook. Familiarize yourself with our policies before signing anything — it makes the remaining forms quick and straightforward.
Complete the Fillable Enrollment Package (on your computer)
Open the PDF in Adobe Acrobat Reader (free) or your browser. Type your child's information directly into the form fields, then save a copy. This package covers child info, developmental history, release consent, medical authorization, and more.
Complete the Policy Acknowledgment & Indemnity
Initial each of the 10 policy items and sign at the bottom. This can be filled on screen (fillable PDF) or printed and initialled by hand.
Sign the Program Hours, Fees & Policies Agreement
Review payment schedule, fee structure, and termination terms. Print, sign, and keep a copy for your records.
Sign the Immunization Acknowledgment
Confirm you understand the vaccine requirements and will provide records before your child's first day. Bring your child's immunization records from their pediatrician.
Optional: Complete the ACH Payment Authorization
If you'd like automatic weekly payments from your bank account, fill out and sign this form. Otherwise, you can pay by check, Zelle, or Venmo each Friday.

Two Easy Ways to Return Your Forms
Drop-In Care Available — Subject to Availability
Need occasional or flexible childcare? We offer drop-in care when spots are open.
⚠️ Availability must be confirmed in advance. Drop-in slots are limited and cannot be guaranteed without prior confirmation. Please call or WhatsApp us at (508) 954-3753 before your intended drop-in date to check availability. The $60 daily rate is a flat fee regardless of the number of hours within the drop-in window.
Getting Ready for Day One
Schedule your tour
After we receive your forms, we'll contact you to arrange a convenient time to visit our home at 25 Louise St. This is a relaxed, informal walkthrough — bring your questions!
Orientation meeting
Before your child's first day, we'll sit down with you to finalize paperwork, discuss your child's daily routine, confirm the start date, and give your child a chance to visit briefly for a smooth transition.
Pay registration & deposit
The non-refundable $75 registration fee and a security deposit equal to one week's tuition are due at this stage to secure your child's spot. Accepted by check (payable to SMARTTECH Early Learning LLC), Zelle, or Venmo.
First Day!
Arrive between 7:30 – 8:00 AM, escort your child to the door, and sign in. We'll take it from there and send you updates throughout the day.
